POLICIES
SOURCE OF PAYMENT
I accept payment only from you, ensuring that I work only for you, not insurance companies or government agencies. We accept any negotiable form of payment or a retainer (monthly or annual). Although I do not participate in managed-care or government healthcare companies, a “SUPERBILL” will be provided to you at the time of service for submission by you to your insurance company for reimbursement. If you cannot afford my published fees, I will first provide the care you need and then make mutually acceptable arrangements for reimbursement.
Giving to the community - because of my practice model I am able to use the revenue of annual fees to subsidize the care of a certain number non-direct care patients.
Pre-Payment Member Fee schedules

New office visit: $125
Follow up office visit: $65
Travel destination consultation and service: $95 plus supplies (inoculations)
Home visit: $199 plus travel time beyond of 10 miles
Corporate on location visit: $199 plus discounted individual charges (multiple persons at location)
Work or school physicals on location visit: $199 plus $45 per individual
Phone, email, text information request: $35 for 15 minute increments
Procedures: by individual procedure with price given before procedure

* To access this service you must be an established patient for obvious reasons
** PROACTIVE HEALTH - your problems list is reviewed on a regular basis whether the you are seen or not to determine if the recent literature contributes to your care. My private data base and search capabilities records active problems and reports to the patient about any applicable advances in treatment. Of course, I welcome any inquiries that my patients have regarding advertisements or news relevant to their condition.
POLICIES
- • Narcotics and controlled substances - These medications are not prescribed on a long term basis except in certain circumstances and these medications are not kept in the office.
- • Walkins and same day appointments - I can occasionally accommodate same day and walkin appointments. I suggest calling the office or going online for e-scheduling to check for availability before traveling to the office. If it is an emergency call 991 or go directly to the emergency room.
- • If you are a woman or a child - you must be accompanied by an adult during your examination.
- • No show for appointment - The first miss - ok, after that a $50 fee.
- • Home or Office Visits - As the circumstance dictates, I can see you in my office, at your home or at your office depending on the medical needs.
- • Payments for services or procedures and tests are to be made at time of service unless prior arrangements are made.
